Student Health Services creates new note policy

Student Health Services is introducing a new process for KU students to document treatment for illness or injury.


The new policy alters the process by which students can request and receive documentation of treatment for illness or injury. The new policy, effective Aug. 1, 2005, will be as follows:


• Students who are incapacitated for three or more consecutive days due to illness or injury can request a Documentation of Illness/Injury statement from the SHS medical professional who either treated the student or received appropriate documentation from another medical provider.


•  Students who have illnesses or injuries of shorter duration and schedule appointments at Student Health Services can obtain a copy of their medical record containing information about their visit by submitting an Authorization for Use/Disclosure form to the SHS Records and Registration Department. The processing of this request will be completed within 10 working days.


The current policy was revised in response to concerns expressed by faculty and staff members.
